Before you begin, you need to have the following documents ready. You begin the registration process without them, but you will not be able to submit it without completing all required fields and uploading the necessary documents.
PROOF OF AGE
- Child’s Birth Certificate
- Child’s Passport
PROOF OF GUARDIANSHIP
- Child’s Birth Certificate
- Adoption Letter
- Legal Custody Documentation *Divorced or separated parents and guardians must provide a copy of a formal court document demonstrating the child’s custody.*
GUARDIAN PHOTO ID
- Driver’s Licence
- Photo ID
- Passport
CURRENT IMMUNIZATIION RECORDS
- Immunizations required are the following:
- DPT (4 doses)
- POLIO (3 doses)
- HIB (1 dose – on or after 1st birthday)
- MMR (1 dose – on or after the 1st birthday)
- Varicella (on or after 1st birthday or proof of disease immunity)
- Pneumococcal Vaccine (PCV) (1 dose – on or after 1st birthday)
- Hepatitis B (3 doses).
PHYSICAL EXAM
- Physical Examination Form - If completed prior to registration, scan and upload during this process, otherwise once complete, please mail to your child's school. New Jersey State Law requires children entering preschool to have CURRENT physical exams that must be dated within the last year.
PROOF OF ADDRESS
As proof of student residency, please provide ONE (1) document listed in category A and ONE (1) documents listed in category B. NOTE: All documents must match the names listed on the birth certificates and all addresses must match the enrolling address (property and mailing address).
- Category A: One (1) of the following documents (must be current)
- Deed
- Property Tax Bill
- Mortgage Statement
- Current Lease (You will only need first page and signature page)
- Affidavit of Domicile (Click here to obtain the form) - If residing with another family who rents/owns a home a notarized Affidavit of Domicile from the owner/renter of the property is needed. (you will still need to provide (2) documents from Category A and B with the owner/renter’s information that signed the Affidavit. Affidavits are only valid for one (1) year.)
- Category B: One (1) of the following documents (must be current)
- Gas, electricity, cable, phone or water bill
- Payroll check/stub
- First class mail/letter from state or federal agency
- Bank statement
- Tax Bill
Prior to registering your child, please complete the following steps below.
Step 1: You MUST have a valid email account to register your child.
Step 2: Review the How To Register manual by clicking here for the Registration Manual or look at this video tutorial of how to register by clicking here. These are meant for you to familiarize yourself with the process.
Step 3: Collect and scan or take pictures of the documents needed they are REQUIRED to register. Click CamScanner for a free scanning app or you can simply take a picture and upload it.
